And a note to our referring providers: Upon sending a patient to Momentum, simply fax the prescription and general patient information and we will obtain any pre-certification that might be necessary and call the patient to schedule the initial evaluation. You may also complete & submit a prescription through this site – Please see the Referring Provider page in the Patient Center.
